CNN’s dump of their latest huge pile of “fake news” is just the latest example of media malpractice. Greta Van Susteren sees it all as out of control:

Greta, at what point do we stop calling these "mistakes" and look at the trend and determine that these are not just innocent mistakes but intended to mislead people.
