President Biden's cognitive issues have been obvious since he took office, and things got even worse this year and everybody saw it. However, Democrats and the media for the first six months of 2024 insisted that Joe was still "sharp as a tack" and dismissed any videos showing otherwise as "cheap fakes."
The Democrats had an election to win, so the press decided to do their thing to help out. MSNBC's Nicolle Wallace was one of them:

Then the debate happened and made things way too obvious. At that point the Democrats decided to show Biden the door and install Kamala Harris as their party's presidential nominee. You know how that worked out for them.

The story of Biden's decline wasn't as much under-reported as it was over covered up. The media worked with Democrats to actively gaslight the country in the hopes they could fool voters about Biden's condition long enough to get them through the election. Then the media teamed up again with the Dems to gaslight the country about Kamala Harris' candidacy and campaign. Fortunately, voters saw through it.

They would "under-report" the story all over again if necessary.
Veteran CBS News reporter Jan Crawford dinged news organizations for not thoroughly covering President Biden’s “obvious cognitive decline” this year until it became unavoidable during his disastrous debate against Donald Trump over the summer.
Crawford, the network’s chief legal correspondent, insisted stronger reporting on the topic could have changed the entire election as she responded to a question from “Face the Nation” moderator Major Garrett about the most underreported story in 2024.
“Undercovered and underreported, that would be, to me, Joe Biden’s obvious cognitive decline that became undeniable in the televised debate,” she said on the Sunday morning show.

“And yet he insisted that he could still run for president. We should have much more forcefully questioned whether he was fit for office for another four years, which could have led to a primary for the Democrats,” Crawford said.
At least somebody finally said it out loud and to their faces.

PolitiFact is still doing their best to memory hole the cover-up about Joe Biden. The true "Lie of the Year" for 2024 was "Biden's fine and it's misinformation to say otherwise," but they instead went with this, via David Marcus at Fox News:
According to readers and editors at PolitiFact, an organization which claims to be a neutral arbiter of truth in politics, the biggest lie of 2024, surely a competitive category, was Donald Trump suggesting that cats and dogs were being eaten by Haitian migrants in Springfield, Ohio.
But I think we can all remember a much, much bigger one.
On Thursday, June 27th of this year, Joe Biden and Donald Trump participated in a presidential debate that not only changed the fate of the country, but exposed the lie of this century, namely that octogenarian Biden was fit as fiddle.
It was with pure shock that America watched its sitting president, life all but drained from his cold visage, barely capable of coming up with words. And at that moment, years of lies about Grandpa Joe’s mental fitness utterly collapsed.
They're still trying to make everybody forget about the gaslighting about Biden. More reporters will probably acknowledge the cover-up effort about Biden now that it's safe to do so. 2024 was the year the media was determined to drive the final nail in the coffin of "journalism," and they succeeded.
